Chaperone Information

Please list in this section the person that will be attending the mandatory apparent welcome meeting on the dancer’s behalf.

Apparel & Costume Information

Costume and Apparel is included in registration fee. Basic costuming items (shoes, tights, leggings) may still be required. Additional information will be provided via email closer to competition. Absolutely no changes will be allowed to sizing/monogram, etc. once registration has been submitted.

Headshot Upload

Please upload your dancer’s headshot to be used in the program (please make sure this is a headshot and not a full body or action shot). No photo will be put in the program if something other than a headshot is submitted. A minimum resolution of 300 dpi is required.

Music Upload

Please upload your DOTY Contestant music here. Please note that ABSOLUTELY NO CHANGES will be allowed to music files. Please pay attention and make sure music file is the correct version.

Required Events

All DOTY contestants are required to attend all convention classes and wear DOTY apparel at all times during classes. They must also attend all opening number rehearsals, participate in the opening number, and perform during the solo semi-finals. Any dancer who is absent from one of these required components will immediately be disqualified from the DOTY competition and entry fees will be forfeited.

Also, the chaperone listed MUST attend the mandatory parent meeting. Any dancer with no representation will be removed from the DOTY competition. Last, there are absolutely no refunds for ANY reason once DOTY registration has been submitted. This includes injury and illness.

As the parent, guardian or teacher of the contestant listed above have reviewed all rules and guidelines regarding the DOTY National Finals competition. You can access the rules and guidelines on our website here: http://www.celebrationtalent.com/nationals-rules.

All information related to the DOTY competition will be sent to the email address listed above.

By digitally affixing my signature, I confirm that I have read and agree to all DOTY rules and guidelines and agree to hold harmless and indemnify Celebration Talent Competition LLC, its agents, directors, employees and independent contractors from any and all claims, suits, liabilities, court costs and legal fees resulting from any injury, damage and/or illness that may occur while participating in any activity related to the competition. I attest that I am an authorized agent or representative of the contestants participating in said competition and hereby authorize the use of their images and names for marketing and advertising purposes.

I further agree that I have read, understand and agree to comply with the Celebration Talent Competition LLC rules and regulations. I understand that title entry fees are due immediately and are non-refundable.
